California requires an itemized pay statement with every payment of wages. Under Cal. Labor Code §226(a), each statement must accurately show: (1) gross wages earned, (2) total hours worked (for hourly employees), (3) the number of piece-rate units and the piece rate if paid by piece, (4) all deductions, (5) net wages, (6) the pay-period dates, (7) the employee’s name and last four digits of the SSN or an employee ID number, (8) the legal name and address of the employer, and (9) every hourly rate in effect during the period plus the hours worked at each rate.
